2008 BOAF Membership Application

Fill out this form to sign-up as a BOAF Member.
The annual membership Dues are:
   $25 for dual
   $20 for single
   $15 senior (over 60)
   $10 for junior (under 18)

Remember to enter the name of the current member who referred you to us. Each member gets a $4 membership discount for each new member they refer.

Your membership becomes active after we have received payment and we have mailed you a BOAF Membership Card.
